What to include

1. Identification of the copyrighted work (URL, title, or description).
2. Identification of the allegedly infringing material on our Site (full URL).
3. Your contact information (full name, address, phone, email).
4. A statement that you have a good-faith belief that use of the material is not authorized by the copyright owner.
5. A statement, under penalty of perjury, that the information in the notice is accurate and that you are authorized to act on behalf of the copyright owner.
6. Your physical or electronic signature.

Counter-notification

If you believe a takedown was made in error, you may submit a counter-notification per 17 U.S.C. § 512(g). Include the same identifying information plus a statement, under penalty of perjury, that you have a good-faith belief the material was removed by mistake.
